The Tick-Borne Diseases Support Group of NJ NY PA is conducting a survey to gather information on the number of people affected by tick borne diseases in the tri-state area. Please take a minute to complete the survey. We value all responses and all responses are anonymous - we do not gather personal information.
The information collected will be tabulated and provided to local and state public health agencies to aid in the fight against tick borne diseases.
Instructions
Your response to the survey is important. Please fill out a survey for each person in your household that is dealing with a tick-borne disease. You can start the SURVEY by clicking the button bellow. SURVEY questions are multiple choice. The survey should take about 2 minutes to complete.
When the SURVEY is completed select SUBMIT at the bottom of the form to record your responses.
